HSI Backs Patient-Centered Primary Care Collaborative (PCPCC)

HealthSciences has joined a collaborative of major employers, consumer groups, patient quality organizations, health plans, labor unions, hospitals, physicians to develop and advance the patient centered medical home. The Collaborative believes that, if implemented, the patient centered medical home will improve the health of patients and the viability of the health care delivery system. As a PCPCC partner, HealthSciences will: 1.) Advocate and advise on strategies and solutions for engaging and preparing physicians, nurses and advanced practice nurses, pharmacists, and other team members, for interdisciplinary and evidence-based patient-centered care, 2.) Recognize and support the formal and volunteer agencies in local communities who offer critical support to primary care teams by providing a safety net to patients with chronic or age-related conditions, and their family caregivers.
