Fourth Edition of the Chronic Care Professional (CCP) Program Released

The CCP Program was developed in response to calls by the World Health Organization and the Institute of Medicine to reshape learning and development in the health professions. HealthSciences has continued to work with organization and association partners to continuously update the CCP program to reflect the latest best practices and evidence-based guidelines for patient-centered chronic disease prevention and care improvement. In June 2008, the Fourth Edition of the CCP was released. Several updates were included in the new program:

  • Updated US and international health care performance data and health care performance improvement solutions.
  • Updated guideline summaries for 25 chronic diseases and age-related conditions from sources including the American College of Cardiology, the American Diabetes Association, the College of Sports Medicine, among others.
  • New sections on the emerging population health improvement movement that encompasses and coordinates employer wellness, and specialty disease management and case management strategies and interventions.
  • More practical approaches and interventions for provider-patient partnership-building including new content on health literacy improvement, cultural competence, and disease self-care and adherence support. A new “whole person care” practice framework was developed to help professionals recognize and better address psychological contributors to health and self-care.
  • The new behavior change facilitation module includes added content on the practical application of brief motivational interviewing in health care settings. A blended five-step brief motivational interviewing and health coaching sequence has been refined and updated based on participant feedback.
  • Expanded evidence-based content on diet and nutrition, obesity and weight management, physical activity, and self-care for professional caregivers. Additionally, in an effort to address growing confusion among purchasers, providers and consumers regarding the terms “health coach” and “health coaching,” evidence-based health coaching standards have been defined.
  • A new web-based CCP eLearning KnowledgeSpace was launched. The new CCP eLearning program is presented in Adobe Flash, with audio, providing web-based learners with a rich learning experience, replicating a live CCP program event.
