Participation in the Chronic Care Professional (CCP) Certification program is funded by organizations committed to preparing health care professionals and interdisciplinary teams for patient-centered chronic disease prevention and care improvement. CCP Evaluation Data

The CCP program is a competency-based learning and development program. As such, evaluation data from individual participants and organization partners has proven key to the success and improvement of the program. In the five years that the CCP program has been offered, over 80% of all tuition fees have been been invested in the continuous improvement and updating of the CCP program.

Each year, CCP evaluation data is reviewed, and improvements and updates made to the curriculum design and program content. Evaluation data sources include:

• On-line and post-live session participant evaluation forms
• Structured post-session and follow-up interviews with participants
• Structured organization post-session and follow-up interviews

The evaluation process has yielded important qualitative and quantitative data. Highlights of this data follow:  

• 90% of organizations who offer the CCP program to their professional staff expand enrollment in the program

• 95% of individual CCP program participants agree or strongly agree that the CCP program has "better prepared" them for their daily job roles

• 98% of individual CCP program participants agree or strongly agree that the CCP program content is relevant to their professional roles

• 94% of individual CCP participants agree or strongly agree that the program is a “good value” for their organizations

• Mean ratings of 4.4 (5.0 scale) have been achieved on all program evaluation domains for both the live and eLearning programs since program inception
